Secondary hyperalgesia, caused by sensitization of dorsal horn neurons, occurs in the undamaged area surrounding the injury. Examples of neuropathic pain include: monoradiculopathies, trigeminal neuralgia, postherpetic neuralgia, phantom limb pain, complex regional pain syndromes and the various peripheral neuropathies. Buprenorphine, a partial opioid agonist and kappa antagonist, has been used as an analgesic and more recently, in combination with nalaxone (Suboxone ®), for the treatment of opioid dependence. A study by Koppert et al. examined the effects of both sublingual and intravenous buprenorphine in hyperalgesia in a randomized controlled trial. After surgery and loss of anesthetic effect, postoperative pain can annoy the patient and affect patient satisfaction with treatment. This study was aimed at evaluating the effect of preoperative pentoxifylline (PTX) on postoperative pain and development of secondary hyperalgesia in patients undergoing laparoscopic appendectomy (LA).

Secondary hyperalgesia refers to the sensitization that occurs because of changes in spinal cord processing. For example, through a process of central sensitization, the firing of dorsal horn nociceptors can change dramatically in the setting of injury (produced by either tissue or nerve damage). Following an injury or cell damage two main things can occur. Primary hyperalgesia and secondary hyperalgesia. Secondary hyperalgesia is indicative of central sensitization. [1] Because secondary hyperalgesia is short-lived, persistent pain after surgery in large part may be primary hyperalgesia, perhaps amplified by central sensitization.
